Job summary

Neighborhood Healthcare PACE is seeking a Referrals Specialist for an on-site, full-time temporary assignment in Riverside, CA. You will coordinate referrals and authorizations, schedule appointments, and help transport participants to care facilities.

The role emphasizes accurate processing, timely follow-ups, and strong communication with providers and families. Ideal candidates will have a High School Diploma or GED, 1 year of healthcare experience, and BLS certification.

Qualifications

• High school diploma/GED required.
• One year of clinical or healthcare experience.
• Experience with referral authorization and data processing preferred.
• Bilingual English/Spanish highly preferred.
• Current Basic Life Support (BLS) certification required.
• CPR and First Aid certification required for all PACE employees.

Responsibilities

• Serve as main point of contact for referrals, authorizations, and appointment scheduling.
• Process and complete referrals accurately and timely, including urgent referrals.
• Arrange transportation for participants to medical appointments.
• Maintain referral logs and update EMR with status notes.
• Assist in re-authorization and follow-ups across all clients and payors.
