Job summary

Qualifications

• Bachelor's Degree in Nursing required; minimum two years of nursing or case management experience.
• Knowledge of nursing assessment methodologies; proficient with Word, Excel, Outlook, PowerPoint.
• Strong communication: read, write, speak English professionally; ability to work under pressure.
• Valid California RN license; BLS certification; valid driver's license.

Responsibilities

• Drive enrollment and refer patients to promote program growth within the site area.
• Educate referral sources on the full spectrum of PACE services and coordinate with physicians, discharge planners, and post-acute leaders.
• Develop strategies to achieve enrollment goals and sustain referral relationships.
• Collaborate with the Interdisciplinary Team from referral to evaluation to ensure a seamless process for participants.
