Job summary

Neighborhood Healthcare in Riverside, CA is seeking a Referrals Specialist to coordinate referrals and follow-up care for PACE participants. This on-site, full-time temporary role lasts about 3 months with a Mon-Fri 8am-5pm schedule.

The position requires experience with referrals, EMR, and patient coordination, plus bilingual English/Spanish is highly preferred. Training on CPT/ICD-10 terms may be provided as needed.

Qualifications

• High school diploma or GED required.
• At least 1 year of clinical or healthcare experience.
• Experience with referral authorization and data processing preferred.
• Bilingual (English/Spanish) highly preferred.
• Current BLS certification required; CPR and First Aid certification required.

Responsibilities

• Acts as main contact for referrals, authorizations, and appointment scheduling.
• Prepares, processes, and completes referrals including urgent and stat referrals.
• Arranges transportation for participants to medical appointments.
• Communicates referral details to participants and families.
• Tracks referrals in logs or EMR and follows up on authorizations.
