Job summary

Premise Health in Foster City, CA is seeking a Full Time Referral Coordinator to support client employees at our health center. The role focuses on coordinating access to medical and social services, documenting encounters in Epic, and collaborating with care teams to keep members on a shared plan of care.

You'll work Mon-Fri 7:30am-4:30pm, engaging with patients, providers, and community partners to improve health outcomes and service delivery, while maintaining a professional, compassionate

Qualifications

High school diploma or GED required.
Experience accessing and navigating community resources preferred.
Strong communication and organizational skills.
Ability to manage caseload and collaborate with teams.

Responsibilities

Identify and coordinate access to medical and social services for members and dependents.
Document encounters and follow-up plans in Epic.
Collaborate with care team and community partners to align on plans.
Schedule appointments and arrange transportation as needed.
Maintain professional, courteous, and respectful engagement with members.
