Job summary

Stamford Health in Stamford, CT is seeking a Referral & Care Coordination Specialist to optimize patient access across Primary Care sites. You will manage new patient scheduling, registration accuracy, and referral processing, ensuring timely care entry and regulatory compliance.

The role requires coordinating with clinical and administrative teams, managing Epic work queues, and maintaining professional communication with patients and offices. On-site schedule is Monday–Friday 7:45 am–4:15 pm.

Qualifications

• High School Diploma or equivalent.
• 3+ years of experience in patient access, centralized scheduling, referrals, and/or registration in a healthcare setting.
• Demonstrated experience managing high-volume phone access and Epic Work Queues.
• Strong organizational, problem solving, and time management skills.
• Excellent verbal and written communication skills.
• Proficiency in Epic, Microsoft Outlook, Word, and Excel.

Responsibilities

• Serves as a primary resource for new patient access across all Primary Care sites.
• Utilizes scheduling systems to schedule appointments per provider templates and protocols.
• Completes and verifies new patient registration with accurate demographics and insurance.
• Resolves scheduling barriers and ensures timely access to care.
• Maintains professional phone interactions aligned with service standards.
• Manages Epic Work Queues (Estimates and Pre-Registration) effectively.
• Documents to support regulatory compliance and minimize billing risk.
• Collaborates with clinical/admin teams to address work queue backlogs.
• Identifies gaps in registration or insurance and resolves them pre-visit.
• Communicates with referring offices to resolve missing information delays.
• Coordinates referrals and initial consultations within specialty lines.
• Supports onboarding and standardization of scheduling workflows.
