Liberation Programs is seeking a compassionate Community Integration Coordinator for the Bridgeport, CT area. This part-time role focuses on assisting individuals returning from incarceration, with emphasis on housing, healthcare access, benefits, and employment readiness.

You will work at Rest Here, a drop-in center offering harm reduction, recovery supports, and workforce development. Strong empathy, organization, and collaboration with partners are essential.

Formación

• Experience with individuals affected by incarceration or related challenges.
• Strong interpersonal and communication skills.
• Ability to provide practical case management and service navigation.
• Knowledge of community resources and barriers for reentry.

Responsabilidades

• Engage individuals returning from incarceration and build trust.
• Identify goals, needs, and barriers; develop reintegration plans.
• Assist with identification, benefits, transportation, and services.
• Provide ongoing case management, advocacy, and follow-up.
• Coordinate referrals to Liberation Programs and partners.
• Support placement opportunities and employment readiness.
• Develop relationships with employers and workforce partners.
• Monitor progress and provide feedback.
