Job summary

Hamilton County in Cincinnati is seeking a Reentry Social Program Specialist to provide care coordination for clients transitioning from incarceration to community living. You will engage community resources, collaborate with service providers, and assess client needs to identify suitable programs and services.

The role supports volunteers, mentors, and a reentry team, with opportunities for growth as the program expands. A bachelor’s degree and relevant experience are required.

Qualifications

• Bachelor’s degree in social work, Human Services, or related field is required.
• Three (3) years’ experience in a social services capacity with direct emphasis on working with disadvantaged population in corrections or treatment environments.

Responsibilities

• Educates, assesses, identify, and requests appropriate community, government or private services that could be needed.
• Conducts case review meetings.
• Case management, review, and documentation.
• Recruits and coordinates volunteers and mentors to work with clients as they transition into a pro-social lifestyle.
• Establishes and maintains a positive relationship with clients, volunteers, mentors, and Reentry team members.

Education

Bachelor’s degree in social work
