Job summary

Shelby County is seeking a Recovery Services Coordinator to support clients with serious mental illness transitioning from the criminal justice system back into the community, under the supervision of the Deputy Administrator - Mental Health Court. Qualifications

• Three years experience in mental health field.
• Bachelor's degree in sociology, social welfare, psychology or closely related field.
• Master's degree preferred.
• Proof of education, training and/or experience required.

Responsibilities

• Develops relationships with local criminal justice system, community services and mental health providers.
• Coordinates resources for adults with mental illness who are incarcerated or at risk of incarceration.
• Identifies, screens and assesses persons with mental illness and facilitates continuity of care while in jail.
• Develops program plans and coordinates transition plans for adults with serious mental illness being released from corrections.
• Collaborates with clients and families to attain recovery goals while avoiding further involvement with criminal justice or hospitals.
• Works with agencies to develop linkage plans for clients.
• Conducts follow-up after release to ensure successful community integration.
• Promotes understanding of needs and rights of mentally ill individuals through education and training.
• Partners with clients to develop an individual plan of services enabling independent living.
• Advocates for individuals to ensure rights and privacy.
