This role serves as the connective tissue between program design and day‑to‑day implementation—supporting staff performance, strengthening service delivery, and ensuring that participant data, case notes, and outcomes align with internal standards, funder requirements, and best practices in trauma‑informed care.

WHAT YOU’LL DO

  • Support staff in developing and implementing high‑quality, individualized service plans aligned with participant goals.
  • Conduct regular check‑ins, case reviews, and supervision meetings to ensure accountability and professional growth.
  • Identify training needs and support ongoing staff development in trauma‑informed care, documentation standards, and service delivery.
Case Management System Oversight & Data Integrity
  • Oversee accurate, timely, and compliant data entry in the organization’s case management system.
  • Review case notes, service plans, assessments, and participant records for completeness, quality, and alignment with internal policies and funder requirements.
  • Ensure staff adherence to documentation timelines, data standards, and confidentiality requirements.
  • Collaborate with leadership to improve workflows, reporting processes, and system utilization.
Program Quality & Compliance
  • Monitor program implementation to ensure fidelity to grant requirements, internal policies, and service models.
  • Support program reporting, audits, and monitoring efforts by ensuring documentation readiness and data accuracy.
  • Identify trends, gaps, or risks in service delivery and recommend improvements to enhance participant outcomes.
Cross‑Functional Collaboration
  • Serve as a key liaison between frontline staff and senior leadership, elevating operational insights and recommendations.
  • Contribute to program planning, evaluation, and continuous improvement efforts.
Community & Organizational Contribution
  • Participate in team meetings, training, and organizational planning sessions.
  • Support a positive, accountable, and mission‑aligned team culture rooted in equity, respect, and transparency.
  • Represent Changing Perceptions professionally in internal and external settings as needed.
WHAT WE’RE LOOKING FOR
  • You have a Bachelor's degree in Human Services or Social Work.
  • You have 3-5 years of experience in a program manager role or equivalent.
  • You have experience supervising case managers or direct service staff in a human services, nonprofit, or reentry‑focused environment.
  • You have a strong working knowledge of case management systems (Apricot, a plus), documentation standards, and data quality oversight.
  • You are highly organized, detail‑oriented, and comfortable holding staff accountable while providing supportive leadership.
  • You are committed to trauma‑informed, equity‑centered, and participant‑driven service delivery.
  • You can balance compliance requirements with compassion and human‑centered leadership.
  • You communicate clearly, professionally, and effectively across diverse teams.
  • You are adaptable, solutions‑oriented, and motivated by continuous improvement.
WHAT YOU WILL GET
