Job summary

Qualifications

• High school diploma, GED/HiSET, or equivalent.
• Experience in human services, recovery support, or related work.
• Knowledge of substance use disorders and recovery resources.
• Ability to build trusting relationships with individuals impacted by substance use.

Responsibilities

• Engage eligible participants before and after release with outreach.
• Complete intake, assess needs, and enroll in reentry and recovery services.
• Develop and update individualized service plans based on goals and needs.
• Maintain regular participant contact and provide ongoing case management and care coordination.
• Coordinate referrals to treatment, housing, employment, health services, and community resources.
• Document participant contacts and progress accurately in required data systems.
