Job summary

Lawndale Christian Health Center is seeking a Recovery Care Coordinator to provide person-centered case management, outreach, and resource navigation for patients with substance use disorders, emphasizing MAR services. • Knowledge of SUD treatment and recovery-oriented care, including medication-assisted recovery, Motivational Interviewing, stages of change, harm reduction, and peer-support principles.
• Ability to build trusting, respectful relationships and provide culturally responsive, trauma-informed, nonjudgmental support to people at all points in recovery.
• Ability to use lived experience and appropriate self-disclosure in a purposeful manner while maintaining professional boundaries and supporting patient choice and agency.
• Ability to recognize and appropriately respond to patient crises and other urgent clinical situations within the scope of the role and organizational policies.
• Strong care coordination, resource navigation, outreach, problem-solving, and patient advocacy skills.
• Strong organizational, analytical, written, verbal, and time-management skills.
• Ability to collaborate effectively within a multidisciplinary healthcare team and with community partners.
• Ability to work independently, demonstrate initiative and sound judgment, and respond effectively to multiple priorities in a fast-paced environment.

Responsibilities

• Provide case management for patients transitioning to or from higher levels of care, including medical stabilization, residential treatment, and recovery homes.
• Coordinate referrals, facilitate timely transitions, and follow up with patients to confirm successful connection to services.
• Monitor MAR patient engagement and outcomes using applicable dashboards and tracking systems.
• Conduct proactive outreach to patients who miss follow-up appointments, and support re-engagement and rescheduling.
• Respond to internal and external referrals for substance use treatment and MAR services, and maintain timely communication with patients, interdisciplinary team members, and referring community partners.
• Support the ongoing development and daily operations of the Recovery Community, including behavioral health groups and recreational activities.

• Use peer-support skills, brief interventions, Motivational Interviewing, and stages-of-change approaches to engage patients in treatment, build rapport, strengthen motivation for change, support individualized recovery goals, and promote retention in care.
• Coordinate medical, psychiatric, behavioral health, specialty, and other appointments for patients participating in recovery services.
• Facilitate warm handoffs and encourage connection to Recovery Community services when appropriate.
• Build and sustain collaborative relationships with community partners.
• Support assertive outreach and linkages to LCHC recovery services, serve as a responsive point of contact for designated partnerships, and cultivate new referral relationships through resource fairs and other community events.
• Maintain timely, accurate, and clinically appropriate documentation of patient encounters in the EHR, including encounters, groups, outreach, and care coordination.
• Provide overdose prevention and harm reduction education and resources.
• Be comfortable administering naloxone, support patient access to naloxone, and manage the supply and distribution of drug-testing kits.
