New North Citizens’ Council, Inc. seeks a Care Coordinator to deliver participant-centered reentry, recovery support, and care coordination in the La Nueva Vida/New Life Program. The role focuses on Black and Latino men impacted by substance use and the criminal legal system.

You will engage participants before and after release, connect them with treatment and community resources, and support goals related to recovery, housing, employment, health, and reintegration.

Qualifikationen

• Strong interpersonal, communication, organizational, and documentation skills are required.
• Ability to maintain confidentiality and appropriate professional boundaries.
• Experience or knowledge relevant to human services, recovery support, reentry services, or related work is preferred.

Aufgaben

• Engage participants before and after release through relationship-based outreach.
• Complete intake, assess needs, and enroll in reentry and recovery services.
• Develop and update individualized service plans based on goals and priorities.
• Maintain ongoing contact and provide case management and care coordination.
• Assist with reentry needs including benefits activation and housing connections.
