Job summary

Catholic Charities Worcester County is seeking a Care Coordinator for the Women’s Recovery Program in Leominster, MA. You will assist residents with aftercare goals, coordinate medical and mental health appointments, and document progress regularly to support continuity of care.

Qualifications include CADC preferred, 1–3 years of experience with substance use, CPR/First Aid, Narcan training, and bilingual Spanish is preferred.

Qualifications

• Minimum 1–3 years of experience with substance use and co-occurring disorders in an ASAM 3.1 level of care.
• CPR-First Aid Certified and Narcan training.
• Bi-lingual in Spanish preferred; ability to write and speak English; strong documentation skills.
• Solid foundation from personal sobriety and active recovery.
• Professionalism, sensitivity, and self-awareness when communicating with residents and staff.

Responsibilities

• Assist residents with aftercare goals and setting up medical/mental health appointments.
• Document monthly or more frequent interaction/follow up sessions.
• Transport residents to appointments and coordinate with external providers.
• Provide guidance and referrals to community services (outpatient services, sober housing).
• Participate in staff meetings, training, and supervision sessions.
• Conduct weekly group therapy sessions focusing on Aftercare Education and resource navigation.
