Job summary

Friends Outside is seeking a WRAP Case Manager to coordinate compliance referrals from San Joaquin County Probation and provide trauma-informed, client-centered case management services. Responsibilities include initiating services, assessing barriers, and developing individualized plans to support probation compliance.

The role emphasizes collaboration with Probation Officers, addressing housing, employment, medical/mental health needs, and reentry support.

Qualifications

• Two-year college degree (or two years’ experience with case management/group facilitation).
• Certified Community Health Worker or able to obtain certification within 12 months.
• Bachelor’s degree in a related field (preferred).
• Bilingual English/Spanish is a plus.

Responsibilities

• Process compliance referrals from probation.
• Initiate communication and services with referred clients.
• Assess and address client challenges to probation compliance.
• Assess client strengths/needs in employment, health, housing, transportation, legal services, education, etc.
• Develop case management plans and monitor progress.
