Job summary

Friends Outside in Tuolumne County is seeking a Senior Social Worker to serve as probation navigator for Prop 47 and Prop 36 referrals, conducting screenings and needs assessments. The role coordinates diversion services, facilitates warm handoffs to community providers, and helps connect youth and families to Medi-Cal, CalFresh, housing, employment, and education supports.

Collaborative work with probation, behavioral health, and schools is essential.

Qualifications

• Master’s degree in social work.
• At least three years of professional experience preferred.
• Experience collaborating with courts and community organizations preferred.
• Grant-funded program experience and outcome reporting preferred.

Responsibilities

• Serve as the primary liaison between Tuolumne County Probation and community-based service providers.
• Coordinate warm handoffs and introductions to improve engagement.
• Provide resource information and ongoing support to youth and families.
• Monitor participation and address barriers to engagement.
• Coordinate referrals to Medi-Cal, CalFresh, and other benefits.
• Schedule appointments and coordinate transportation.
• Support employment, housing, and education program participation.
• Ensure compliance with program requirements and maintain documentation.
• Maintain trauma-informed, client-centered practices in all interactions.
