Job summary

Fortune Society is seeking a Transition Coordinator to support recently incarcerated individuals transitioning to the community from Rikers Island and NYS facilities. The role involves managing a caseload, evaluating client needs, and coordinating referrals for wrap-around supports.

Qualifications

• High School Diploma/GED is required.
• Some college or equivalent professional experience preferred.
• Experience with individuals with legal system involvement is preferred.
• Experience with documentation, record keeping and data entry is required.

Responsibilities

• Conduct transition plans with individuals currently incarcerated and complete service plans after return to the community.
• Have a weekly presence on Rikers Island, at least twice per week, and assist with program information sessions.
• Provide referrals to supportive services and maintain regular follow-up with participants and providers.
• Manage a caseload of up to 75 individuals and conduct ongoing case management and outreach.
• Maintain proper documentation of participant files and complete case notes in a timely manner.
• Attend community events to promote engagement for participants and potential enrollees.
