Job summary

Centurion Health is seeking a Transitional Healthcare Facilitator for Indiana Women's Prison in Indianapolis.

This full-time role develops and implements transitional healthcare plans for incarcerated individuals scheduled for release and coordinates with the Indiana Department of Correction and other agencies to ensure pre-discharge planning and connecting re-entering individuals with needed services and resources.

Qualifications

• Bachelor’s degree in human services, social services, or criminal justice field.
• Two years of experience in a correctional setting.
• Experience with community and state programs and services.
• Two years of re-entry or release planning experience preferred.
• Ability to pass background investigation and obtain security clearance.

Responsibilities

• Develop and implement transitional healthcare plans for individuals scheduled for release.
• Coordinate with Indiana Department of Correction and state/local agencies for pre-discharge planning.
• Provide resources and services to help re-entering individuals.
• Schedule shifts across Indiana Women's Prison and Heritage Trails Facility as needed.
