Job summary

Centurion Health seeks a full-time Transitional Healthcare Liaison to develop and implement transitional healthcare plans for incarcerated individuals scheduled for release. Collaboration with the Indiana Department of Correction ensures pre-discharge planning is completed for re-entering society.

Role requires two years in a correctional setting, knowledge of community programs, and CPR/AED certification. Travel within Indiana is required. Benefits included with full-time employment.

Qualifications

• Bachelor’s degree in psychology, social work, counseling or related field from an accredited institution required
• Two (2) years of experience working in a correctional setting
• Experience with/knowledge of community and state programs and services
• Two (2) years of experience in re-entry services or release planning preferred
• Must show/obtain current PPD tuberculosis screening documentation
• Must be appropriately and actively certified in CPR and AED use
• Must be able to pass background investigation and obtain agency security clearance where applicable

Responsibilities

• Develop and implement transitional healthcare plans for incarcerated individuals scheduled for release
• Coordinate with Indiana Department of Correction and other agencies to ensure pre-discharge planning
• Visit other juvenile facilities within Indiana as part of short term travel requirements
