Job summary

Centurion Health seeks a Transitional Healthcare Facilitator at Indiana Women's Prison in Indianapolis to develop and implement transitional healthcare plans for incarcerated individuals scheduled for release. You will coordinate with the Indiana Department of Correction and local agencies to ensure pre-discharge planning is completed.

The role requires a Bachelor's degree in a human services field and at least two years of experience in a correctional setting, with knowledge of re-entry

Qualifications

• Bachelor's degree in human services, social services, or criminal justice required.
• At least two years of experience in a correctional setting.
• Experience or knowledge of community and state programs and services.
• Two years of re-entry or release planning experience preferred.
• Must maintain TB screening documentation and CPR/AED certification.
• Must pass background check and obtain agency security clearance.

Responsibilities

• Develop and implement transitional healthcare plans for individuals scheduled for release.
• Coordinate with the Indiana Department of Correction and other agencies to ensure pre-discharge planning.
• Assist re-entering individuals in accessing services and resources.
