Job summary

Centurion Health is seeking a Transitional Healthcare Facilitator for Indiana Women's Prison in Indianapolis.

This full-time role develops and implements transitional healthcare plans for incarcerated individuals scheduled for release and coordinates with the Indiana Department of Correction and other agencies to ensure pre-discharge planning and connecting re-entering individuals with needed services and resources.

Qualifications

• Bachelor’s degree in human services, social services, or criminal justice field.
• Two years of experience in a correctional setting.
• Experience with community and state programs and services.
• Two years of re-entry or release planning experience preferred.
• Ability to pass background investigation and obtain security clearance.

Responsibilities

• Develop and implement transitional healthcare plans for individuals scheduled for release.
• Coordinate with Indiana Department of Correction and state/local agencies for pre-discharge planning.
• Provide resources and services to help re-entering individuals.
• Schedule shifts across Indiana Women's Prison and Heritage Trails Facility as needed.

Pay Rate $25 per hour + benefits!Centurion is proud to be the provider of comprehensive healthcare services Indiana Department of Correction.We are seeking a Transitional Healthcare Facilitator for needs at Indiana Women's Prison in Indianapolis, Indiana. This position will typically be primarily scheduled at Indiana Women's Prison with at least one day per week at Heritage Trails (or more) depending on number of weekly releases at each site.The Transitional Healthcare Facilitator will be responsible for the development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release. The Transitional Healthcare Facilitator will coordinate with the Indiana Department of Correction and other state and local agencies to ensure that pre-discharge planning and preparation is accomplished, and those re-entering society are provided with necessary services and resources.Bachelor’s degree in human services, social services, or criminal justice fieldTwo years of experience working in a correctional settingExperience with/knowledge of community and state programs and servicesTwo years of experience in re-entry services or release planning preferredMust maintain current tuberculosis screening documentation and active CPR and Automated External Defibrillator (AED) use certificationMust be able to pass background investigation and obtain agency security clearance where applicableAvailable Shift:Full-time days, Monday-FridaySchedule is split, typically 4 days per week at Indiana Women's Prison and 1 day (or more) at Heritage Trails Correctional Facility (subject to change depending on number of weekly releases at each site)Centurion is the provider of healthcare services for the Indiana DOC.
