Job summary

Centurion Health is seeking a full-time Transitional Healthcare Facilitator at Rockville Correctional Facility in Rockville, IN. The role focuses on developing and implementing transitional healthcare plans for inmates scheduled for release and coordinating with state agencies to ensure successful re-entry and access to needed services.

The position requires a Bachelor's degree in psychology, social work, counseling or related field, with at least two years' experience in a correctional setting

Qualifications

• Bachelor's degree in psychology, social work, counseling or related field required.
• Two (2) years of experience in a correctional setting preferred.
• Experience with community and state programs and services.
• Two (2) years of experience in re-entry services or release planning preferred.
• Must pass background check and TB screening (PPD).
• Must be CPR and AED certified.

Responsibilities

• Develop and implement transitional healthcare plans for incarcerated individuals scheduled for release.
• Coordinate with Indiana Department of Correction and other agencies to ensure pre-discharge planning and re-entry services.

Education
