Job summary

State of Colorado is seeking a Community Programs Specialist III to support parolee re-entry efforts in the Denver region. The role requires coordinating case management, building community resources, and collaborating with multiple agencies to enhance public safety.

The position focuses on developing transition plans, delivering services, and maintaining detailed records to monitor progress and outcomes. Applicants should have significant experience in correctional or human services settings

Qualifications

• Six (6) years of professional experience with hard-to-serve/high-needs clients in correctional/human services.
• Experience with offender management components (ICC, classifications, job placement, etc.).
• Ability to locate housing, jobs and community resources; conduct regular client meetings.
• Education transcripts may be required to meet qualifications.

Responsibilities

• Assess re-entry needs, barriers and strengths using formal and informal tools.
• Develop individualized transition plans based on assessed needs.
• Deliver case management and instruction in housing, employment and pro-social skills.
• Facilitate group modules using evidence-based curriculum and technology.
• Provide transitional services for high-needs and homeless in release meetings.
• Collaborate with agencies to connect individuals with re-entry resources.
• Maintain and analyze data, and document records for re-entry planning.
• Attend multi-agency staffing to plan and implement Re-Entry goals.
• Perform other duties as assigned.
