Job summary

Concordance in Richmond, VA is seeking a Post-Release Case Manager to guide participants through the program, deliver 1:1 and group sessions, and coordinate with the care team to ensure continuity of care. The role emphasizes trauma-informed practices and connecting participants to essential services.

You will work within the Case Manager department, support center-wide events, document progress, and uphold the program curriculum while fostering participant growth and resilience.

Qualifications

• Bachelor’s degree in a social service-related field (social work, psychology, counseling, criminal justice, sociology, or similar).
• CPR/First Aid preferred, or ability to complete within first 90 days.
• Strong communication and interpersonal skills with compassion for others.
• Familiarity with and endorsement of 12-step programming.

Responsibilities

• Conduct 1:1 and group sessions focusing on life skills and growth.
• Deliver reflections and trauma-informed care when needed.
• Connect participants to community resources (housing, healthcare).
• Document sessions in the electronic records system and Reflections forms.
• Collaborate with the Care Team and participate in meetings and trainings.
