Job summary

Delphi Rise, part of Delphi Drug & Alcohol Council Inc., seeks a full-time case manager to support individuals returning to the community from incarceration in Monroe County. The role focuses on dignity, relationship-building, and helping clients access housing, employment, health care and healing services.

Responsibilities include managing 40–50 clients, coordinating with state and county agencies, maintaining records, developing service plans, and facilitating support groups.

Salary: $24.50-$27.50/hour

Job Summary

This position provides person-centered case management services to individuals returning to the community from incarceration, as part of Delphi Rise’s Monroe County Reentry Program. Our approach is grounded in dignity, respect, and belief in each person’s potential. We walk alongside clients as they rebuild their lives, offering support with housing, employment, health care, and healing.

Through our reentry program, we support individuals in creating a future defined by stability, purpose, and hope.

Essential Duties and Responsibilities

Additional duties may be assigned.

  • Manage a caseload of 40 to 50 clients, supporting access to benefits, employment, treatment, housing, childcare, healthcare, identification documents, and other critical services.
  • Coordinate services in collaboration with the New York State Department of Corrections and Community Supervision (Parole), Monroe County Probation, local courts, and community partners.
  • Maintain accurate and timely case notes and client records.
  • Develop individualized service plans that align with each client’s goals and needs.
  • Facilitate or arrange support groups such as Cognitive Behavioral Therapy, parenting education, domestic violence support, and anger management.
  • Conduct biweekly follow-up contacts with all assigned clients.
  • Actively participate in Monroe County Reentry Task Force meetings.
  • Provide services at the Probation Office and in Monroe County Jail 1-2x per week.
  • Perform other duties as assigned by management.

Supervisory Responsibilities

This role does not include supervisory responsibilities.

While we strongly encourage applicants with lived experience in the criminal justice system, candidates cannot have been on probation within the past three years. Access to the Monroe County Jail is an essential part of this role, and all candidates must meet the jail’s clearance requirements.

Knowledge, Skills, and Abilities

  • Ability to build rapport and effectively engage individuals with serious mental health challenges or criminal justice involvement.
  • Strong written and verbal communication skills.
  • Excellent organizational skills with the ability to manage multiple priorities.
  • Analytical thinking to assess needs, prioritize actions, and solve problems.
  • Familiarity with community resources that meet diverse client needs.
  • Ability to work independently while contributing positively to a team environment.
  • Cultural humility and comfort working in diverse environments.
  • Adaptability and a good sense of humor.
  • Proficiency with computers, including web-based data systems.
  • Valid driver’s license with a clean driving record. Occasional travel required for meetings, appointments, and community events.

Work Environment & Physical Demands Equipment Used

Regular use of computers/iPads, printer, copier, scanner, fax, phone, shredder, and calculator. Software includes Microsoft Office: Word, Excel, Outlook, PowerPoint, and Access.

Mental and Visual Demands

Moderate attention required for administrative tasks, data entry, and client documentation. Some analysis and decision-making needed within established procedures.

Physical Requirements

The role involves frequent sitting, standing, walking, talking, hearing, and repetitive hand motions. Occasional pushing or pulling. No heavy lifting (under 5 pounds).

Environmental Conditions

Some off-site work may be required to meet with clients or attend community events. This may include exposure to inclement weather or travel to partner agency locations.
