Job summary

Oklahoma Department of Mental Health and Substance Abuse Services seeks an Integrated Services Discharge Manager in Lexington, OK (Joseph Harp Correctional Center). The role links individuals leaving correctional facilities to mental health and substance abuse services, housing, and recovery supports.

You will manage a caseload, collaborate with an interdisciplinary team, and screen and refer to statewide reentry programs; travel may be required to fulfill job-related responsibilities.

Qualifications

• Master's degree in a behavioral health field and one year of experience in professional case management behavioral health services.
• Bachelor's degree in a behavioral health field and two years of professional experience in professional case management behavioral health services.

Responsibilities

• Manage a caseload and work as part of an interdisciplinary team.
• Screen and refer individuals to state-wide reentry programs.
• Coordinate with social benefits and educational assistance entities.
