Job summary

452 MENTAL HEALTH AND SUBSTANCE ABUSE SERV. seeks an Integrated Services Discharge Manager to coordinate linkage to mental health, SUD services, housing, and recovery support for individuals leaving correctional facilities.

You will manage a caseload, work with an interdisciplinary team, and collaborate with social benefits and educational agencies to screen and refer clients to reentry programs across the state.

Qualifications

• Master’s degree in a behavioral health field and one year of experience in professional case management behavioral health services OR a bachelor’s degree in a behavioral health field and two years of professional experience in professional case management behavioral health services.

Responsibilities

• Link individuals exiting correctional facilities to mental health and substance abuse services, housing, and recovery support in the community.
• Manage a caseload and collaborate with an interdisciplinary team including correctional staff.
• Screen and refer individuals to state-wide reentry programs and social benefits/educational resources.
