Job summary

State of Washington seeks a Psychiatric Social Worker 3 (PSW3) at the Washington Corrections Center in Shelton, WA. This full-time day-shift role (Sat/Sun off) collaborates with internal and external partners to plan medical, behavioral health, and substance use transition services for incarcerated individuals.

Responsibilities include release planning, reentry assessments, trauma-informed care, and coordinating with DDA/DSHS and MCO to ensure continuity of care upon release.

Qualifications

• Must be able to obtain and maintain status as an Agency Affiliated Counselor through the Washington State Department of Health or possess a current, valid WA Social Work license.
• Incumbent must apply for agency affiliated counselor license within 7 days of hire.
• Must have a current, valid Driver’s License.

Responsibilities

• Conduct release planning for incarcerated individuals with healthcare follow up needs and coordinate community resources.
• Complete Reentry Needs Assessment and Health Services Reentry Plan.
• Facilitate reentry groups and set follow-up medical and mental health services in the community.
• Coordinate care with DDA, DSHS HCS, and MCO; participate in MDT/FRMT meetings and case conferences.
