Position: HCA Re-entry Coordinator

Salary Range: $4,887.00 /month to $6,548.00/month, Starting Salary: DOQ, DOE

Location: County Corrections Facility

Employment Type: Non-Rep Grade 5, Full-time (40 hours/week), Benefits Apply, FLSA Nonexempt

Brief Duties and Responsibilities:

The Reentry Coordinator takes a lead role in coordinating program services to assist Medicaid eligible incarcerated clients with their transition back into the community through a collaborative partnership with in-house staff, community partners embedded in our corrections facilities, as well as community providers of medical, and behavior health services. The position affords the opportunity to direct Medicaid clients into services designed to promote their healthiest lives, deliver Health Related Social Needs, and navigate reentry with the goal of successful transitions from jail to the community, thereby reducing recidivism.

Working Environment/Physical Abilities:

Work is performed inside or outside of a corrections facility. Position will have limited contact with inmates. Physical abilities required for this position are ones typically related to office operations, including manual dexterity and visual acuity to operate personal computers and other office equipment, access filing system, and have sufficient hearing and speech ability to communicate verbally.

Minimum Qualifications: Education and Experience:

Bachelor#39;s Degree in one of the Social Sciences, Criminal Justice, or a closely related field from a college of university recognized by the US

Department of Education AND two (2) years of progressively responsible professional experience in service program planning and administration OR any equivalent combination of experience and education that provides the applicant with the desired knowledge, skills and ability required to perform the work. Ability to obtain within six (6) months of hire WA State Patrol ACCESS Level II certification.

Licenses and other Requirements:

A valid driver's license and a driving record acceptable to the county's insurer required. Maintain current CPR, first aid. Successfully pass independent personal and background investigation.
