Job summary

Centerstone in Columbus, Indiana is seeking a Recovery Specialist to provide support services and wellness coaching in both community-based and office settings. You will help clients develop their Wellness Recovery Action Plan (WRAP) and coordinate services to promote recovery and quality care.

You will conduct intake, assessments and discharge planning, assist with transportation, and collaborate with families and providers to remove barriers and support sustainable wellness outcomes.

Qualifications

• Must be at least 21 years old per funding requirements.
• Associates degree required; Bachelor’s degree preferred.
• Two years of relevant experience in human services.
• WRAP facilitator certification within 12 months preferred.

Responsibilities

• Provide group or individual training focused on improving functioning and health.
• Assist clients in developing Wellness Recovery Action Plans (WRAP) and wellness goals.
• Empower clients to self-manage chronic conditions and navigate care systems.
• Link clients to community resources and collaborate with providers.
• Conduct intake, assessments, and discharge planning with follow-up.
• Provide transportation assistance to support wellness goals.
