Job summary

Community Health Resources (CHR) in Putnam, CT, is seeking an APRN for its Mobile OTP program. This part-time role (25 hours/week) involves 4 hours daily, Monday through Friday, with morning shifts.

The APRN will provide MOUD services, clinical evaluations, and community care within a SAMHSA-certified mobile unit and Putnam OTP base. Responsibilities include direct and indirect clinical services, case management collaboration, and staff supervision as needed.

Qualifications

• 5 years' experience in adult or child mental health.
• CT APRN license is required.

Responsibilities

• Provides administration of medications as indicated under direction of the agency MD.
• Provides treatment services consistent with payors and clinical standards.
• Maintain positive relationships with community health organizations and outside medical services.
• Provide training and consultation to staff as indicated.
• Develop programs and psycho-educational interventions for clients across the agency.
• Participates in case conferences and team meetings as needed.
• Makes comprehensive bio-psycho-social assessments and develops treatment focus.
• Creates clinically appropriate, focused treatment plans and reviews.
• Provides individual, group and/or family therapy as appropriate.
• Assess level of care needs and refer or adjust treatment length as needed.
• Provides brief treatment for clients without major mental illness.
• For clients with serious mental illness, provides acute treatment as indicated.
• Demonstrates competency in age-related or specialty issues for each population served.
