Responsibilities

• Document in EMR and bill for services.
• Screen patients for substance use and determine level of care.
• Provide care management and navigation to access treatment services and community resources.
• Coordinate referrals to primary care, withdrawal management, and therapy.
• Maintain patient progress notes and coordinate with insurance aftercare.
