High Point & Affiliated Organizations in Plymouth, MA, seeks a Recovery Support Navigator to help clients as they transition from treatment to community-based care. The role requires a Bachelor's degree or lived recovery experience, a valid driver's license, and the ability to work with diverse populations.

Travel with mileage reimbursement is provided. You will perform CSP case management, coordinate with outpatient units, maintain HIPAA compliance, complete documentation and billing, and

Qualifications

• Bachelor's degree or lived experience of substance abuse and recovery.
• Valid driver's license and ownership of a car.
• Preference given to those with at least one year of related job experience.

Responsibilities

• Comply with Federal Regulations, HIPAA, 42 CFR Part 2, Confidentiality of Alcohol and Drug Abuse Patients.
• Maintain safety and security of client/staff at all times.
• Provide client-centered case management CSP services including transportation and linkage to medical/counseling appointments.
• Coordinate with HPTC outpatient units in region for client education and sign-ups for CSP.
• Provide 24 hour on-call coverage in program rotation.
• Accurately complete and submit required billing sheets and expense reports in required time frame.
