High Point & Affiliated Organizations is seeking a Recovery Support Navigator for Community Based Recovery Services in Hyannis, MA. The role requires a Bachelor's degree or lived recovery experience and a valid driver's license with a car. You will provide client-centered case management, coordinate CSP program activities, and collaborate with diverse providers.

Travel is required and mileage reimbursement is provided. Full-time position with 8am–4:30pm shifts.

Qualifications

• Ability to work collaboratively with clients from diverse backgrounds including family members and providers
• Maintain healthy boundaries
• Knowledge of confidentiality policies and procedures
• Knowledge of basic office skills (phone, fax, copying, filing)
• Ability to work independently within a team approach
• Strong verbal, written and organizational skills
• Knowledge of basic computer skills and EMR systems

Responsabilités

• Ensure compliance with Federal Regulations, HIPAA and 42 CFR Part 2
• Follow program policies and procedures including ethics and boundaries
• Maintain safety and security of clients and staff at all times
• Notify Program Director/Authorities in case of crisis or emergency
• Attend mandatory trainings and meetings
• Provide client-centered case management services (transportation, referrals, appointments)
• Coordinate with outpatient units for CSP program sign-ups and education
• Coordinate monthly regional group activities and 24-hour on-call coverage
• Use Motivational Interviewing and Recovery Coaching in all encounters
• Document progress and complete required billing and reporting
