Job summary

High Point & Affiliated Organizations is seeking a Recovery Support Navigator to join its Community Based Recovery Services (CBRS) team in Brockton. The role involves client-centered case management, coordination with outpatient units, and supporting recovery as clients transition back into their communities.

You will collaborate with diverse stakeholders, maintain confidentiality, document progress, and participate in trainings. A Bachelor’s degree and a valid driver’s license are required.

Responsibilities

• Provide client-centered CBRS services as indicated in needs assessment/service plan.
• Coordinate with administrative staff for authorization requests.
• Document needs assessments, service plans and encounter notes in required time.
• Maintain safe and secure environment for clients and staff at all times.
• Utilize Motivational Interviewing and Recovery Coaching in all client encounters.
• Complete required billing sheets and expense reports on time.
• Coordinate with outside providers to enhance quality of care.
• Attend mandatory trainings, staff meetings and supervision sessions.

Pay Rate: $28.20 - $33(Experience and Education dependent)
Status: Full Time
Shift: Monday - Friday 8am - 4:30pm
Travel required & includes mileage reimbursement

High Point provides a range of community-based services including Community Based Recovery Support (CBRS). CBRS was created to provideadditional support to people as they work on their recovery within their communities. Often, clients who have been in an addiction treatment or Section 35 facility and are transitioning back into their communities' benefit from extra support.

Recovery Support Navigator Requirements

• Ability to work collaboratively with alcohol/drug dependent clients from diverse legal, social, economic, ethnic, cultural and racial backgrounds as well as family members, insurance providers and outside providers;
• Possess the ability to maintain healthy boundaries;
• Knowledge of appropriate confidentiality policies and procedures;
• Knowledge of basic office skills: phone, fax machine, copy machine, filing;
• Knowledge of basic computer skills and software including Microsoft Word/Excel;
• Ability to work independently without direct supervision while adhering to the program’s philosophical team approach;
• Certification in HPTC’s approved de-escalation program as well as CPR;
• Participation in annual TB screening.

Recovery Support Navigator Responsibilities

• Compliance with Federal Regulations, HIPAA, 42 CFR Part 2, Confidentiality of Alcohol and Drug Abuse Patients;
• Compliance to and working knowledge of all applicable HPTC policies and program procedures including but not limited to those relating to ethics/boundaries and duties of role;
• Maintain safety and security of client/staff at all times;
• Ensure timely contact of Clinical Director in event of a crisis/emergency;
• Attend all mandatory trainings, staff meetings, and supervision meetings as required;
• Provide client-centered case management CBRS services as indicated in needs assessment/service plan (temporary transportation, 12 step meetings, linkages to medical/counseling appointments, court etc.),
• Provide coordination with HPTC outpatient units in region for client education/approach/sign-ups for CBRS program, following Initiation policy;
• Coordinate monthly regional “group activity”
• Utilize Motivational Interviewing and Recovery Coaching in all client encounters;
• Maintain appropriate level of productivity/billable hours;
• Coordinate with administrative assistant for all authorization requests;
• Collaborate with outside providers as indicated;
• Accurately complete/document needs assessments, service plans, encounter notes, and service delivery reports in required time;
• Accurately complete all required billing sheets and expense reports in required time frame;
• Accurately document all client progress on required forms in required time frame;
• Investigate and develop community contacts and cooperative networks with other community groups and agencies;
• Coordinate with other departments and facilities to enhance and ensure quality care for all clients;
• Cooperate with other health and social service agencies in an effort to increase community services;
• Must become a certified Recovery Coach within 1 year of employment:
• Other duties as assigned by the Clinical Director/Program Director.
