Job summary

Boston Medical Center (BMC) is seeking a Recovery Support Navigator to provide care management and navigation for patients in the substance use treatment system. You will coordinate referrals, assist with insurance navigation, and support access to therapy and primary care within Project ASSERT.

The role emphasizes teamwork with a multidisciplinary health care team, cultural sensitivity, and strong documentation in the EMR.

Qualifications

• Bachelor’s degree (or equivalent) preferred.
• Degree in social work, psychology or related human services field is highly preferred.
• Associate’s degree (or equivalent combination) plus a minimum of 1-2 years in healthcare is preferred.

Responsibilities

• Document in the electronic medical record and ensure data entry, data integrity, and data tracking compliance including billing.
• Screen patients for substance use and determine level of care using TAPs and other screeners.
• Provide care management and navigation to engage patients and access treatment services and community resources.
• Assist patients in locating and establishing substance use treatment services and coordinating referrals to primary care, withdrawal management, and therapy.
• Monitor patient progress with functioning, adherence to treatment plans, and service needs.
• Schedule outpatient visits and follow-up as needed.
• Educate and support clients with culturally and linguistically appropriate strategies; coordinate with Interpreter Services.
• Build relationships with community resources to ensure patient access and promote collaboration among the care team.
• Document encounters and interventions with date/time/signature in EMR; bill for encounters accordingly.
• Maintain compliance with policies, procedures, and data reporting requirements; adapt to departmental needs.
