Job summary

Boston Medical Center is seeking a Recovery Support Navigator within Project ASSERT to provide care management and navigation for patients with substance use disorders, coordinating referrals to primary care, withdrawal management, and therapy while guiding insurance issues. The RSN will document encounters in the EMR, screen for level of care, arrange outpatient visits, and collaborate with a multidisciplinary team.

Strong communication and empathy are essential.

Qualifications

• Bachelor’s degree (or equivalent) preferred.
• Degree in social work, psychology or related human services field is highly preferred.
• Associate's degree (or equivalent) plus a minimum of 1-2 years in healthcare is preferred.

Responsibilities

• Documentation in electronic medical record; data entry and billing.
• Screen patients for substance use to determine level of care.
• Provide care management and navigation support to engage patients in care and access services.
• Coordinate referrals to primary care, withdrawal management, and therapy; assist with insurance issues.
• Monitor patient progress and follow up on care plans and needs.
• Schedule outpatient visits and follow-ups as needed.
• Educate and counsel clients; collaborate with Interpreter Services when needed.
• Build relationships with community resources to improve patient access.
• Collaborate with multidisciplinary health care team to promote prevention and harm reduction.
• Document encounters and interventions with time/date and signature.

Education

Bachelor’s degree preferred

Social work/psychology/related field preferred

Associate’s degree + 1-2 years healthcare experience preferred
