Job summary

Boca Recovery Center in Springfield, MA is seeking a compassionate Client Care Coordinator to guide clients from admission through discharge and into aftercare. You will coordinate aftercare plans, assist with referrals to outpatient programs, sober living, and housing, and support AMA prevention and case management tasks.

You will lead group sessions on relapse prevention, collaborate with outreach, clinical, operations, and alumni teams, and ensure accurate EHR documentation while delivering

Qualifications

• Associate's degree in a behavioral health field is required.
• Experience in behavioral health or substance use treatment preferred.
• Strong communication and documentation abilities, with group facilitation skills.

Responsibilities

• Develop and implement individualized aftercare plans with clients and clinical teams.
• Coordinate referrals to outpatient programs, sober living, and recovery housing.
• Lead relapse prevention and life skills group sessions for ongoing recovery.
• Assist with FMLA, disability documentation, and other case management tasks.
• Ensure seamless care coordination across clinical, outreach, alumni, and operations.
