Job summary

UMass Memorial Health Milford Addiction Care is hiring a Recovery Support Navigator to provide care management and system navigation for patients with substance use disorders. The RSN engages patients as they present in the treatment system, supports access to treatment services and community resources, and helps set goals and recovery plans in collaboration with the care team.

Required: high school diploma and valid driver's license; preferred bachelor's in social work or psychology; strong

Qualifications

• High school diploma or equivalent is required.
• Valid driver's license is required.
• Bachelor's degree in social work, psychology, or related field is preferred.

Responsibilities

• Develops goals and objectives with the patient based on identified needs.
• Ensures coordination with the patient's care plan and current providers.
• Connects the patient with providers to develop and implement a comprehensive care plan.
• Educates patient about treatment options and helps access community supports.
• Aids in identifying resources for families and recovery supports.
