Job summary

Spectrum Health Systems, Inc. in Norfolk, MA seeks a Recovery Support Navigator to engage, educate and support patients seeking treatment upon release and their families.

The role begins pre-release and continues after return to the community, focusing on coordinating services for a successful re-entry. Responsibilities include managing a caseload, documenting all activities within required timeframes, and planning discharge with or without medication while ensuring continuity of SUDT services.

Qualifications

• High school diploma required; LADC-II preferred.
• Bachelor's degree in human services or related field or equivalent experience preferred.
• Case management experience strongly preferred.

• Beginning pre-release, and continuing after release and return to the community.
• Managing an assigned caseload and completing all required documentation within established timeframes.
• Discharge planning addressing initiation/continuation of SUDT services, with or without medication.
• Coordinating a re-entry package and documenting receipt of nasal naloxone.

Location: Norfolk, MA - Norfolk Pondville Massachusetts Correctional Institution

Schedule: Monday - Friday, 8:00am-4:00pm

Pay rate: $23.90-$27.51/hour with Bachelors degree

The Recovery Support Navigator Is Responsible For

• Providing non-clinical services to engage, educate and support patients seeking treatment upon release and their families in order to motivate and maintain them in culturally relevant behavioral health services in the community
• Beginning pre-release, and the support continues after the patient is released and returns to the community
• Managing an assigned caseload and completing all required documentation for services as required by Spectrum, DOC and all licensing, accreditation and regulatory bodies within established timeframes
• Discharge planning that addresses initiation and/or continuation of SUDT services, with or without medication for addiction treatment
• Coordinating a re-entry package for each patient scheduled for release, and documents receipt of nasal naloxone

Qualifications

• High school diploma required and LADC-II preferred
• Bachelor's degree in human services or related field or equivalent experience in the behavioral health field preferred
• Case management experience strongly preferred
