Job summary

Spectrum Health Systems is seeking a Recovery Support Navigator at Souza Baranowski Correctional Center in Shirley, MA. The role provides clinical case management to engage patients seeking opioid use disorder treatment starting up to 120 days pre-release and continuing up to 90 days post-release in the community.

The position requires a high school diploma with LADC-II preferred and may include bachelor’s level candidates with related behavioral health experience.

Qualifications

• High school diploma required and LADC-II preferred.
• Bachelor’s degree in human services or related field or equivalent experience in the behavioral health field preferred.
• Case management experience strongly preferred.

Responsibilities

• Providing clinical case management to engage, educate and support patients seeking treatment upon release and their families to motivate and maintain them in culturally relevant behavioral health services in the community.
• Working independently and as a team
• Assisting patients with transportation and housing resources and make referrals as needed
• Working on-site unless they are in the community working with patients.
• Coordinating necessary treatment for various patient needs and communicating with all referral sources and treatment entities using proper releases of information
• Working hours required to meet the business needs of the organization and travel to other programs or locations as needed

The Recovery Support Navigator is responsible for
