Job summary

Paycom is seeking a Recovery Support Navigator in Norfolk, MA to provide non-clinical services for patients seeking treatment and their families, beginning pre-release and continuing support after release.

The role manages a caseload, completes required documentation for Spectrum, DOC and licensing bodies, and coordinates discharge planning for SUDT services, including nasal naloxone documentation, in a community-based setting.

Qualifications

• High school diploma required; LADC-II preferred.
• Bachelor's degree in human services or related field or equivalent experience preferred.
• Case management experience strongly preferred.

Responsibilities

• Providing non-clinical services to engage, educate and support patients seeking treatment upon release and their families in order to motivate and maintain them in culturally relevant behavioral health services in the community
• Beginning pre-release, and the support continues after the patient is released and returns to the community
• Managing an assigned caseload and completing all required documentation for services as required by Spectrum, DOC and all licensing, accreditation and regulatory bodies within established timeframes
• Discharge planning that addresses initiation and/or continuation of SUDT services, with or without medication for addiction treatment
• Coordinating a re-entry package for each patient scheduled for release, and documents receipt of nasal naloxone
