Job summary

Spectrum Health Systems, Inc. in Gardner, MA and Concord, MA seeks a Recovery Support Navigator to engage and support patients seeking treatment upon release, coordinating non-clinical services and community resource connections to sustain engagement in behavioral health care.

From pre-release through community reintegration, you will manage a caseload, complete required documentation, and contribute to discharge planning for continued SUDT services, with a focus on culturally relevant support

Qualifications

• High school diploma required.
• Bachelor's degree in human services or related field preferred.
• LADC-II certification preferred.
• Case management experience strongly preferred.

Responsibilities

• Provide non-clinical services to engage, educate and support patients seeking treatment upon release.
• Begin pre-release support and continue after patient returns to the community.
• Manage an assigned caseload and complete required documentation within designated timeframes.
• Discharge planning addressing initiation/continuation of SUDT services, with/without medication.
• Coordinate a re-entry package for each patient scheduled for release and document receipt of nasal naloxone.

Location: Gardner, MA and Concord, MA - North Central Correctional Institution and Northeastern Correctional Center

Schedule: Monday - Friday, 8:00am-4:00pm

Pay rate: $23.90-$27.51/hour with Bachelors degree

The Recovery Support Navigator is responsible for:

• Providing non-clinical services to engage, educate and support patients seeking treatment upon release and their families in order to motivate and maintain them in culturally relevant behavioral health services in the community
• Beginning pre-release, and the support continues after the patient is released and returns to the community
• Managing an assigned caseload and completing all required documentation for services as required by Spectrum, DOC and all licensing, accreditation and regulatory bodies within established timeframes
• Discharge planning that addresses initiation and/or continuation of SUDT services, with or without medication for addiction treatment
• Coordinating a re-entry package for each patient scheduled for release, and documents receipt of nasal naloxone

Qualifications

Qualifications:

• High school diploma required and LADC-II preferred
• Bachelor's degree in human services or related field or equivalent experience in the behavioral health field preferred
• Case management experience strongly preferred
