Job summary

Spectrum Health Systems, Inc. is hiring a Recovery Support Navigator for the Shirley, MA MOUD program at Souza Baranowski Correctional Center. The role involves engaging patients before release and supporting them through the transition to community-based behavioral health services.

Responsibilities include coordinating treatment, assisting with transportation and housing resources, and collaborating with referral sources. Flexibility for travel and on-site work is expected.

Qualifications

• High school diploma required; LADC-II preferred.
• Bachelor’s degree in human services or related field preferred.
• Case management experience strongly preferred.

Responsibilities

• Provide clinical case management to engage, educate and support patients seeking treatment upon release and their families to motivate and maintain them in culturally relevant behavioral health services in the community.
• Working independently and as a team
• Assisting patients with transportation and housing resources and make referrals as needed
• Working on-site unless they are in the community working with patients.
• Coordinating necessary treatment for various patient needs and communicating with all referral sources and treatment entities using proper releases of information
• Working hours required to meet the business needs of the organization and travel to other programs or locations as needed

Education

High school diploma

Bachelor’s degree in human services or related field

LADC-II preferred

Schedule:

Full-time. Hours and days are flexible

Pay rate:

$23.90-$26.51/hour

The Recovery Support Navigator Is Responsible For

• Providing clinical case management to engage, educate and support patients seeking treatment upon release and their families to motivate and maintain them in culturally relevant behavioral health services in the community. This engagement begins 120 days pre-release, and the support continues up to 90 days post-release to the community


• Working independently and as a team


• Assisting patients with transportation and housing resources and make referrals as needed


• Working on -site unless they are in the community working with patients.


• Coordinating necessary treatment for various patient needs and communicating with all referral sources and treatment entities using proper releases of information


• Working hours required to meet the business needs of the organization and travel to other programs or locations as needed
