Job summary

The Recovery Support Navigator at Spectrum is responsible for providing non-clinical services to engage, educate and support patients seeking treatment upon release and their families in order to motivate and maintain them in culturally relevant behavioral health services in the community.

Support begins pre-release and continues after release, while managing a caseload and ensuring documentation meets Spectrum, DOC and regulatory standards within set timeframes.

Qualifications

• High school diploma required and LADC-II preferred.
• Bachelor’s degree in human services or related field or equivalent experience in the behavioral health field preferred.
• Case management experience strongly preferred.

Responsibilities

• Providing non-clinical services to engage, educate and support patients seeking treatment upon release and their families.
• Beginning pre-release, and continuing support after release.
• Managing an assigned caseload and completing required documentation for services with regulatory bodies.
• Discharge planning addressing initiation/continuation of SUDT services, with or without medication for addiction treatment.
• Coordinating a re-entry package for each patient scheduled for release and documenting naloxone receipt.

Schedule:Monday - Friday, 8:00am-4:00pm
Pay rate:$23.90-$27.51/hour with Bachelors degree

The Recovery Support Navigator is responsible for:

• Beginningpre-release, and the support continues after the patient is released and returns to the community


• Managingan assigned caseload and completing all required documentation for services as required by Spectrum, DOC and all licensing, accreditation and regulatory bodies within established timeframes


• Discharge planning that addresses initiation and/or continuation of SUDT services, with or without medication for addiction treatment


• Coordinatinga re-entry package for each patient scheduled for release, and documents receipt of nasal naloxone
