Job summary

Groups Recover Together in Bedford, Texas seeks a Recovery Support Coordinator to guide 160-180 members through recovery, coordinating clinical, social, and community care. The role blends in‑person and remote support to optimize engagement and wellness throughout the recovery journey.

You will work with a multidisciplinary team, provide coaching and resource navigation, and help members access housing, transportation, and healthcare services while coordinating care pathways and documentation.

Qualifications

High school diploma required; an Associate’s degree in Social Services is preferred.
Two+ years in case management, care coordination, or peer support encouraged.
Experience with electronic health records (EHR) and community resources is beneficial.
State-specific qualifications and certifications may be required depending on location.

Responsibilities

Collaborate with a multidisciplinary care team to ensure high‑quality, member‑centered care.
Build trusted relationships and support members in achieving recovery goals through coaching and motivational interviewing.
Identify goals, provide coaching, and engage members in recovery activities.
Assist unengaged members to reengage in treatment through outreach.
Navigate members through insurance, housing, transportation, and community resources.
Maintain shared caseloads and coordinate care pathways with the team.
Manage referrals, ROIs, and documentation to facilitate care transitions.
