Qualifications

• MD Board-approved qualifications or trainee statuses (CAC-AD, LCPC, etc.) or equivalent per MD Board rules.

Responsibilities

• Maintain RESTORE services visibility and availability within Bartlett.
• Oversee day-to-day administration and implementation of RESTORE.
• Serve as liaison between RESTORE team, patients, and Bartlett clinic staff.
• Maintain patient tracker/registry and ensure timely documentation.
• Prepare for and facilitate weekly interdisciplinary treatment team meetings.
• Document activities in EMR and registries; support clinic resources.
