Job summary

Community Health Resources is seeking a Residential Aide for the Heartwood Home program in Willimantic, CT. The role supports residents in a structured recovery environment, providing day-to-day living skills, meal preparation, hygiene, budgeting, and personal care within a 24-hour staffed setting.

The position requires a High School Diploma or equivalent, experience in Human Services, and a valid driver’s license with a good driving record.

Qualifications

• High School Diploma or equivalent.
• Experience in Human Services with responsible work history.
• Valid driver's license with good driving record; CPR & First Aid Certification required OR must obtain within 90 days of employment.

Responsibilities

• Maintains physical environment and program standards to CHR, DMHAS, JCAHO and licensing standards.
• Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.
• Effectively communicates day’s events to other staff.
• Teaches and assists residents in rehab areas: nutrition and healthy living, meal preparation, hygiene, laundry, budgeting, and basic home care skills.
• Assists/Manages administrative functions and tasks at supervisor direction.
• Prepares and facilitates educational and skill building groups and one-to-one interventions as directed by the treatment plan.
• Collaborates and coordinates care with providers and families as needed.
• Assists residents in addressing medical and psychiatric needs.
• Maintains documentation in compliance with agency and program standards.
• Participates in staff meetings, supervision, trainings and other meetings as assigned.
• Additional duties as required.
