Job summary

(CHR) Community Health Resources, Inc. seeks a Residential Aide for Heartwood Home in Willimantic, CT.

This full-time night position (Mon–Fri, 11:00 PM–7:30 AM) supports residents in a 12-bed recovery program, with 24-hour staffing to promote stability and growth. Qualifications include a High School Diploma or equivalent, experience in human services, and a valid driver’s license with good record; CPR/First Aid certification or ability to obtain within 90 days.

Qualifications

• Education: High School Diploma or equivalent.
• Experience: Experience in Human Services with responsible work history.
• Licensure/Certification: Valid driver’s license with good driving record; CPR & First Aid certification required or must obtain within 90 days of employment.

Responsibilities

• Maintains physical environment and program standards per CHR, DMHAS, JCAHO and licensing standards.
• Monitors medications of all residents per policy.
• Transfers day’s events to staff using appropriate communication methods.
• Teaches/residents in rehab areas: nutrition, healthy living, meal prep, hygiene, budgeting, basic home care.
• Assists with administrative tasks and supports treatment plans.
• Prepares/facilitates educational and skill-building groups and one-to-one interventions.
• Coordinates care with providers and families; addresses medical/psychiatric needs.
• Maintains documentation per agency standards.
• Participates in staff meetings, trainings, and other meetings.
