Job summary

Community Health Resources is seeking a Residential Aide for Heartwood Home in Willimantic, CT. This full-time role supports a structured recovery program, with a nightly schedule from 11:00 PM to 7:30 AM and Monday–Friday shifts.

The position emphasizes frontline care, supervision, and coordination with staff to assist residents with daily living skills, budgeting, nutrition, medication monitoring, and crisis support within a licensed residential setting.

Qualifications

• Education: High School Diploma or equivalent.
• Experience in Human Services with responsible work history.
• Valid driver’s license with good driving record. CPR & First Aid Certification required OR must obtain within 90 days of employment.

Responsibilities

• Maintains physical environment and program standards to CHR, DMHAS, JCAHO and licensing standards.
• Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.
• Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the day’s events.
• Teaches and assists residents in rehab areas such as nutrition and healthy living, meal preparation, hygiene, laundry, budgeting, and basic home care skills.
• Assists/Manages administrative functions and tasks at the direction of the supervisor.
• Prepares and facilitates educational and skill building groups and one-to-one interventions per treatment plan.
• Collaborates and coordinates care with providers and families as needed.
• Assists residents in addressing medical and psychiatric needs.
• Maintains documentation in compliance with agency and program standards.
• Participates in staff meetings, supervision, trainings and other meetings as assigned.
• Additional duties as required.
