Job summary

Qualifications

Responsibilities

• Maintains physical environment and program standards.
• Monitors medications of all residents.
• Transfers information to staff regarding the day’s events.
• Teaches residents in rehab areas: nutrition, healthy living, budgeting, and basic home care skills.
• Prepares and facilitates educational and skill-building groups and interventions.
• Collaborates with providers and families as needed.
• Maintains documentation in compliance with agency standards.
